A case of advanced rectal carcinoma associated with chronic idiopathic thrombocytopenic purpura. Significance of high-dose intravenous .GAMMA.-globulin as pre-operative treatment.

From the age of 21, the patient had been suffering from chronic idiopathic thrombo-cytopenic purpura. The patient underwent splenectomy at the age of 47, and has been treated by corticosteroids for over twenty years. Rectal carcinoma was found at the age of 59 by barium enema examination. Despite these treatments for ITP, platelet count was 2.1×104⁄mm3. on admission. Therefore, on the basis of Imbach's report, the patient received 0.4gr of intact γ-globulin on three consecutive preoperative days. High-dose intravenous γ-globulin induced a dramatic initial increase in platelet count: the count rose to 22×104⁄mm3. on the operative day. Under this treatment, the patient was able to undergo radical rectal resection without any trouble during operation. High-dose intravenous γ-globulin seems to be effective for patients with ITP who develop surgical disorders.
