A New Treatment Method by Submucosal Ethanol/Steroid(E/S) Injection for Allergic Rhinitis : A Pilot Study

We examined the effect of mixture of ethanol/steroid(E/S) on allergic rhinitis by the submucosal E/S injection therapy that has been developed by us. All the cases before visiting our department showed high antibody levels to the house dust, mites and cedar pollen and have received some sort of medical treatment during 2/3 or longer in a year in other medical facilities. A dispersed submucosal injection of 0.6 to 1.0 ml of the E/S mixture was performed into the inferior nasal turbinate of these patients. The rhinoscopic examination in the patients before treatment confirmed the bilateral obstruction of inferior nasal meatus due to the bilateral swelling of inferior turbinate mucosa and a large volume of nasal secretion. The total symptom score(TS score) of nasal symptoms over all cases was 6 to 15 (11.2±2.8) before the treatment. The total examination score(TE score) of intranasal local findings was 3 to 6 (8±0.9). The TS scores after the initiation of treatment were 4.4±2.3 in Week 1, 1.7±1.3 in Week 4, 1.2±1.5 in Week 12, 1.3±1.1 in Month 6, 3.5±3.2 in Year 1(n=19 thus far), 2.2±1.5 in Year 2(n=14), 4.3±2.9 in Year 3(n=7). These values over the observation period were statistically significant in comparison with those before the treatment (P<0.001). The values of TE scores of intranasal findings after the injection were 2.9±0.9 in Week 1, 1.5±0.8 in Week 4, 0.9±0.9 in Week 12, 0.9±0.9 in Month 6, 1.9±1.6 in Year 1(n=19 thus far), 1.9±0.9 in Year 2(n=4), 2.7±2.0 in Year 3(n=7). All of these values were significantly lower than those before the injection (P<0.001). No administration of other drugs was required in 7 of 19 cases, and the medication score significantly decreased from 3.5± 0.5 before the administration to 0.7±0.6 after the treatment (P<0.001). Lesion in the mucosal surface was slight and neither open ulcer nor postoperative infection developed. The results described above confirm the very high effectiveness of the present injection for the treatment of allergic rhinitis, the safety of treatment even in a narrow region like the nasal mucosa, the pain alleviation due to operation in patients, and the large contribution to the saving of total medical payment.
