Stent Graft for Thoracic Aortic Aneurysm Performed with Abdominal Aortic Surgery

To investigate efficacy of stent grafting for thoracic aortic aneurysm performed simultaneously with abdominal aortic surgery. Seven patients (4 men and 3 women) of thoracic aortic aneurysm associated with abdominal aortic diseases (6 aneurysms and 1 arteriosclerosis obliterans) underwent thoracic aortic stent grafting simultaneously performed with abdominal aortic graft replacement surgery. Elective surgery was performed in 6 cases and emergency surgery in 1 case. While simultaneous procedure was planned before surgery in 5 cases out of the 7 cases, changing to the simultaneous procedure was decided during surgery in the remaining 2 cases. In the latter cases, an access artery was too small to perform thoracic aortic stent grafting. The mean time of the surgery in the 5 cases where the simultaneous procedure was preliminarily planned was 343 minutes. Average amount of bleeding was 1,150g. Although collapse of stent graft was observed in one case, there was no paraplegia or death associated with the simultaneous procedure in the present study. The stent grafting for thoracic aortic aneurysm performed simultaneously with abdominal aortic surgery is useful for securing thick access route even in the patients who are small in stature and can be implemented safely.
