Selective Platelet Removal as a Novel Therapy for Refractory Crohn's Disease

Blood platelets are best known for their essential role in haemostasis, but are suspected to contribute to a variety of inflammatory disorders as well. Therefore, we thought that selective depletion of peripheral platelets might be a novel and safe therapy for Crohn's disease (CD). Here we report a patient with refractory CD successfully treated by selective platelet removal therapy (SPRT) utilizing a commercially available centrifugal device. A 28-year-old male who had a diagnosis of ileo-colitis CD with CD activity index (CDAI) 242.8 was admitted to our hospital to receive SPRT. The study time course consisted of a 24-week treatment and 4 weeks of follow up. In total, 7 monthly SPRT sessions were performed during the treatment. The device was set to remove approximately 4.0×10^<11> platelets during each 90-min session. Treatment efficacy was assessed by measuring CDAI and patient's quality of life (QoL) according to a standard inflammatory bowel disease (IBD) questionnaire (IBDQ). On average, 32.5% of his peripheral platelets were removed during each SPRT session without removing a significant number of white blood cells. There was no significant improvement in CDAI or IBDQ, but there was a significant fall in the main inflammation markers, C-reactive protein (CRP; p<0.02) and erythrocyte sedimentation rate (ESR, p<0.02) after each SPRT session. In conclusion, platelet reduction might be a novel approach to achieve anti-inflammatory effect in IBD. Further studies in large cohorts of patients are warranted to fully evaluate the therapeutic efficacy of SPRT.
