Effects of Isoflurane and Midazolam on Hemodynamics when Combined with Thoracic Epidural Anesthesia

We compared the effects of isoflurane and midazolam on hemodynamics when epidural anesthesia was combined with each of these anesthetic agens in 20 patients undergoing gastric resection. Following thoracic epidural anesthesia, 10 patients received general anesthesia with 60% nitrous oxide and 0.4% isoflurane (group 1), and another 10 patients with 60% nitrous oxide and 0.1 mg/kg midazolam (group 2). After the induction of both epidural and general anesthesia, heart rate (HR) and mean arterial pressure (MAP) significantly decreased in both groups. Cardiac index decreased to 70±5% (p<0.01) of the preanesthetic value in group 1 and 72±5% (p<0.01) in group 2. In group 1, MAP was maintained at low values throughout the surgery. However, in group 2, MAP recovered to the preanesthetic value 60 min after the beginning of surgery. The catecholamine dose administered to maintain cardiac index during surgery was significantly greater in group 1 than in group 2. This data shows that epidural anesthesia combined with 0.4% isoflurane induces a greater decrease in MAP, and needs a larger volume of catecholamines to maintain cardiac index.
