Coronary Artery Vasospasm Induced by Tracheal Suction in a Patient under Thoracic Epidural Anaesthesia

A 77-year-old man without history of angina pectoris underwent subtotal gastrectomy under thoracic epidural anaesthesia supplemented with nitrous oxide and sevoflurane. The surgical procedure was uneventful. Inhalation of sevoflurane was discontinued and the suction tube was introduce into the trachea tube. Immediately after this procedure marked hypotension, bradycardia and ST-segment elevation were observed. Suspected coronary artery vasospasm was relieved by nitroglycerin and vasopressor agents. Three hours after the attack, echocardiogram revealed no abnormal wall motion in left ventricle. It was most likely that coronary artery vasospasm was triggered by tracheal suction and developed with increased vagal tone under thoracic epidural anaesthesia.
