D-Penicillamine Treatment in Rheumatoid Arthritis

Out of 51 patients with definite and classical rheumatoid arthritis (RA) treated with D-Penicillamine (D-Pa), 43 were treated for more than 6 months (27.4±13.8 months) and 8 for less than 6 months (3.7±5.0 weeks) due to adverse effects. The activity of RA was assessed retrospectively by the number of active synovitis on the method of seven day variability for the responsiveness of D-Pa. Thirty eight out of 43 patients (88%) had some benefits with D-Pa and 27 (63%) had prominent benefits. The patients responsive to D-Pa had the tendency to have many active synovitis and rheumatoid nodules. In the responsive cases, the number of active synovitis was decreased even with low dose D-Pa (100-200 mg/day) within 2 months, and the remission has been maintained for a long time. Positive anti-nucleal antibody (ANA) was detected in 29% of cases with adverse effects and 16% without it. ANA test was useful to predict the adverse effects.
