Displacement of the maxillary first molar in malocclusion during clenching

Movements of the maxillary first molar in normal occlusion have been measured previously to clarify part of the mechanism of oral function. However, movements in malocclusion have not yet been measured. Therefore, we measured the two-dimensional displacement of the maxillary first molar in 13 patients with malocclusion during clenching, and explored its possible association with occlusal characteristics. The following results were obtained : 1. Twelve of the 13 maxillary first molars, which made contact with their opposing molars at the same sites in each clenching, showed the same movement pattern in each of three measurements. 2. The magnitude of displacement ranged from 9 to 139μm. 3. Eleven of the 13 molars studied were displaced towards the lingual side, whereas 2 were displaced buccally. On the other hand, 8 molars moved distally and 5 moved mesially. These phenomena depended on the balance of occlusal forces exerted on both sides of the center of the molar's occlusal surface. 4. The path of molar displacement was either monophasic (without fluctuation) or multiphasic (fluctuating at at least one point). Of the 13 maxillary first molars studied, four were in a cusp-to-cusp relationship with their antagonistic molars and had multiphasic paths. The present study showed that the molar displacement during clenching in malocclusion was not consistently different from that in individual normal occlusion, but rather varied.
