The prevalence of TMJ osteoarthrosis in orthodontic patients at Fukuoka Dental College Hospital

The purpose of this study was to examine the prevalence of TMJ osteoarthrosis (CA) in orthodontic patients, and to investigate the characteristics in TMJ symptoms and radiographic findings. The sample consisted of 322 orthodontic patients, 104 males and 218 females, who were going to start orthodontic treatment at the Orthodontic Clinic of Fukuoka Dental College Hospital. They had no congenital deformities. We examined TMJ symptoms and radiographic findings by questionnaires, interviews, clinical examinations, and panoramic radiographs. The results were as follows. 1. The prevalence of TMJ dysfunction was 35.4% (28.8% in males and 38.5% in females). 2. The TMJ CA patients were nineteen out of the total patients and the prevalence was 5.9% (1.9% in males and 7.8% in females). 3. Unilateral TMJ CA cases were thirteen and bilateral cases were six. 4. For radiographic findings, a condylar deformity was found in nine cases and condylar osteophyte was found in ten cases. Six cases showed condylar erosion, and condylar flattening was found in 14 cases. 5. In unilateral CA patients the condylar ratio of the affected side tended to be smaller than that of the unaffected side, but in older patients with more than 30 years of age, the difference between both sides was small. 6. For the clinical findings, TMJ sound was recognized in all CA patients, however, the patients with TMJ pain and limited mouth opening were rare. The incidence of TMJ CA in orthodontic patients, 5.9%, found in this study suggested that we must carefully examine clinical signs and panoramic radiographs taken during orthodontic treatment in order to find out early the morphologic changes of the condyle.
