Pineoblastoma Showing Unusual Ventricular Extension in a Young Adult : Case Report

MRI上特異な進展形式を示した松果体芽腫の一例を報告した. 症例は19歳男性,主訴は頭痛, 神経学的には両側鬱血乳頭を認めた. CTにて石灰化を伴う松果体部腫瘤, 閉塞性水頭症を認めた. MRIにて松果体部に主座を置く腫瘍は中脳水道を介して第四脳室内まで浸潤していたが, 腫瘍に接する周囲の構造物の形態は保たれ, 圧迫所見を認めなかった. 神経内視鏡下に腫瘍生検, 第三脳室底開窓術を行った. 組織診断は松果体芽腫であり, 術後全脳全脊髄照射を施行し, 画像上腫瘍の完全消失を認めた. 松果体芽腫は周囲を圧迫しながら浸潤発育し, 高率に髄腔内播種を認める. これまでに周囲の構造を保ちながら第四脳室内にまで進展した報告は認めなかった.
