Subarachnoid Hemorrhage from Intracranial Dissecting Aneurysms of the Anterior Circulation : Two Case Reports

クモ膜下出血で発症した2例の頭蓋内テント上解離性動脈瘤を報告する。最初は56歳の女性で左中大脳動脈近位部の解離性動脈瘤の症例である。手術は左中大脳動脈基始部の閉塞と左浅側頭動脈―中大脳動脈吻合術を施行したが予後は良くなかった。2例目は61歳、男性で左前大脳動脈近位部の解離性動脈瘤の症例である。手術は親血管が開存するようにクリッピングを行い、さらに筋膜片で補強した。術後経過は良好である。明らかな嚢状動脈瘤がみとめられないクモ膜下出血の患者では内頚動脈系の解離性動脈瘤も鑑別診断として考慮にいれなければならない。
