Intracerebral Infiltration by Monoclonal Plasmacytoid Cells in Waldenstrom's Macroglobulinemia : Case Report

Waldenstrom's macroglobulinemiaは、血液中にIgMを産生する形質細胞が腫瘍性に増殖する疾患である。この疾患の脳内転移は非常に稀で、Bing-Neel syndromeと名付けられた。我々の症例は68才女性で3年前にWaldenstrom's macroglobulinemiaの既往がある。記憶障害を主訴に来院。MRIのT_2強調像にて左前頭葉及び脳染にT_2延長病巣を認め同部は造影剤(Gd-DTPA)にて増強された。生検にて確定診断がなされた。腫瘍細胞はVirchow Robin腔の形質細胞と脳実質内の未分化なリンパ球より構成され、前者はIgM及び入軽鎖抗体陽性で、後者は陰性であった。Monoclonalな増殖はSouthern blot analysisにて証明された。放射線治療にて腫瘍は消失し、3年間再発していない。
