Hemodynamic Evaluation of the Effect of Percutaneous Transluminal Angioplasty for Atherosclerotic Disease of the Vertebrobasilar Arterial System

椎骨脳底動脈灌流不全43例に対するPTA前後の局所脳循環動態の変化について報告する。43例は計51の狭窄性病変を有し、内訳は椎骨動脈起始部17、頭蓋内椎骨動脈32、脳底動脈2であった。脳循環動態は10mg/kg acetazolamide負荷前・負荷後^99mTc-HMPAO-SPECTで評価した。PTA後臨床的に改善を示した24例中18例がPTA前にsubnormal perfusion, また20例がacetazolamide負荷後の血管反応性の低下を認めた。また椎骨動脈起始部の単一狭窄症例12例中1例のみに症状の改善を認めた。一方、頭蓋内病変あるいは多発性病変31例中23例に症状の改善を認めた。以上より、PTAの適用は脳循環不全状態を呈する後者にあると考えられた。
