Embolization of Cerebral Aneurysms Using Guglielmi Detachable Coils : Problems and Treatment Plans in the Acute Stage after Subarachnoid Hemorrhage and Long-term Efficiency

GDCを用いた破裂脳動脈瘤の急性期治療の問題点と長期治療効果について検討した。くも膜下出血後2週間以内に治療を行った急性期治療群8例で手技による合併症は認めなかった。5例は経過良好で3例は死亡した。2例は脳血管攣縮による広汎な脳梗塞、1例は肺炎が死亡原因であった。治療後9ヵ月以上経通観察が行えた長期治療群は14例16動脈瘤であった。small neckを有するsmall aneurysm 6例全例で完全閉塞が達成でき再開通は認めなかった。5動脈瘤でcoil compactionによる再開通を認めた。GDCは破裂動脈瘤の急性期治療としての再破裂予防効果が朋待できた。しかし、coil compactionによる再開通は重要な問題であり、治療後の厳重な観察が必要であった。
