A NUCLEAR DNA STUDY OF UTERINE CERVICAL DYSPLASIA WITH REFERENCE TO ITS PROGNOSTIC SIGNIFICANCE

Chicago大学産婦人科外来より得られた Uterine cervical dysplasia症例の Papanicolaou染色細胞診標本に対して High Resolution Cytometry (TICAS)を用いた核DNA解析より, dysplasia予後推定の可能性および疫学的背景について retrospective studyを行つた. 1962〜1985年の間に dysplasiaと診断された2,526例のうち, 259例を本実験の対象とし, これらを progressive群, persistent群, regressive群の3群に分けて検討した. 核DNA解析は, それらのうち81例で行なわれ, 以下の結果を得た. 1. 初回妊娠の低年齢, 多妊多産, 炎症の合併症および pillの服用等を背景にもつ若年の症例は, progressionの頻度が高く, その期間も短い. また, そのseverityに関係なく, 2〜3ヵ月以内の再検と, 2.5〜3年追跡が必要であると思われた. 2. 核DNA解析より, dysplasiaの異型度が高い症例は, aneuploidy areaにstem lineをもつ型が多かつた. また, progressive群と他の2群との判別予測は可能であると思われ, progressionに関する best nuclear indexは, mean nuclear areaと思われた.
