Twenty-eight hips in 25 patients with early arthrosis resulting from acetabular dysplasia were treated with shelf operation. Age ranged from 19 to 45 years (average, 28.8 years) at the time of operation. Mean follow-up period was 5.8 years. The centre-edge angle of Wiberg improved from a preoperative mean of -6.2° to 29.3°. Progression of osteoarthritis was seen in 5 hips and the other 23 showed excellent remodeling. The mean hip score assessed by the criteria of the Japanese Orthopaedic Association improved from 74.1 points preoperatively to 92.4 at 2 years later. Fair or poor results of less than 80 points were obtained in 10 hips at the final follow-up. Age over 30 years and an advanced stage of osteoarthritis were linked to an unsatisfactory outcome. We conclude that excellent results can be obtained for the patients under the age of 30 years and patients over the age of 30 years are not good candidates for shelf operation when the affected hips are in an advanced stage.
