A large intake of animal protein is considered to be one of the important risk factors in renal calcium stone formation. We found that patients with stones can be classified into two groups acccording to their response to a 40 g dietary animal protein loading ; one group showing a remarkable increase in urinary excretions and the other showing little change. There seems to be a difference in the acid-base balance between these two groups. In patients whose arterial bicarbonate ion concentration greatly decreased after the protein loading, urinary excretions changed significantly. On the other hand, in patients whose arterial bicarbonate ion concentration increased or changed little after the loading, there was little change in urinary excretions. These results suggest that this difference in response to acid loading may be an important risk factor in urinary stone formation.
