A Case of Ileus Induced by Transient Ischemic Colitis After Laparoscopic Sigmoidectomy

In October 2008, a woman in her 50s underwent electromagnetic radiation (EMR) for early–stage sigmoid colon cancer. Cancer cells had invaded to the submucosal layer invasion in 3,000μm deep and the surgical margin was positive. Following referral for further surgery, she underwent laparoscopy in December 2008 to dissect the superior rectal artery immediately after the left colic artery branch, and the distal colon was cut at the promontrium. Reconstruction used double stapling because anastomotic blood flow presented no problems. Surgery took 180 minutes, and blood loss was 14 g. The postoperative course was uneventful and the woman was discharged on postoperative day (POD) 8. She was rehospitalized 15 days there of ter for ileus. Abdominal computed tomography (CT) indicated ischemic descending–colon colitis 10 cm from the anastomotic area. Conservative management relieved symptoms, and the woman was discharged on POD 53 after rehospitalization. Although laparoscopic surgery complications are rarely reported, the possibility should be considered in such surgery as our case.
