We report a case of metacronous paraaortic lymph node metastasis six years after transverse colon carcinoma resection. A 53–year–old woman who had undergone transverse colon resection with regional lymphadectomy including group 3 lymph nodes in May 2002 was found in pathological examination to have moderately differentiated adenocarcinoma (type 3, ss, ly0, v2, n1, ow–, aw–, ew–). She did not undergo adjuvant chemotherapy due to chronic hepatitis. CT showed paraaortic lymph node swelling in November 2008, and PET showed high uptake in the paraaortic lymph node but more elseiwhere. CEA was 9.8 ng/ml. She was diagnosed with paraaortic lymph node metastasis, undergoing 30 courses of modified FOLFIRI (mFOLFIRI). CT showed lymph node metastasis had shrunk and no other metastasis was detected. CEA dropped to 2.0 ng/ml.
