Pancreatic Tumor Invasion T Staging in Advanced Gastric Cancer with MDCT

Background: Pancreatic invasion in advanced gastric cancer strongly influences the treatment course was considered when computed tomography (CT) indicates fat layer obliteration between the tumor and pancreas. We focused on the contrast difference between the tumor and pancreas in enhanced CT to evaluate the value of contrast in advanced gastric cancer.<BR>Materials and Methods: We evaluated 36 T4a(SE) and T4b(SI) gastric cancer cases based on CT and operative findings, comparing conventional CT versus enhanced contrast CT in diagnosing pancreatic invasion.<BR>Results: Conventional CT diagnosed 15 T4a(SE) tumors without fail but not 21 of pancreas invasion. We separated these 21 into clear interface (15) and unclear (6) by contrast. Of the 15 clear interface cases, 14 (93.3%) did not have pancreatic invasion and all unclear cases were positive for pancreatic invasion, confirmed by surgery.<BR>Conclusions: Contrast may enable more efficient, safer treatment to be selected preoperatively in subjects with advanced gastric cancer.
