Mammography is a useful modality for breast cancer screening in women over 50. For women under 50, the group in which the incidence of breast cancer is the highest, mammography is less useful because cancers can be hidden by dense breast tissue. Ultrasound would be useful in this age group for several reasons. First, ultrasound is effective regardless of breast density, and there have been many reports of ultrasound being used to detect impalpable breast cancers. Second, ultrasound examination of breasts is already popular, so the necessary equipment is widely available.
However, there are some problems associated with using ultrasound in screening. A protocol has not yet been established : for example, which kind of equipment should be used and who should perform the examination-doctors or screening personnel ? How do we record the variety of data obtained from ultrasound ? These issues need to be resolved.
Because of its sensitivity, ultrasound can produce false positive readings. This problem can, to some extent, be addressed by setting up agreed criteria for assessing lesions. Overall, ultrasound is considered to be a useful modality for breast cancer screening, especially in women under 50y.
