Breast-Conserving Therapy for Breast Cancer Patients Showing Mammographic Evidence of Calcification.

Between August 1992 and February 1996, 37 breast cancer patients with mammographic evidence of calcifications were treated by conservative surgery. These patients were classified into two groups-linear type and granular type-according to the shape of the microcalcifications on the mammograms. We evaluated the prognosis, EIC, surgical margin positivity and histopathological findings. The linear type was significantly (p <0.01) more likely to be accompanied by the comedo type than by the non-comedo type. There were two different types of histological calcifications the necrotic type and the psammomatous type. The necrotic type was more likely to be accompanied by the comedo type than by the non-comedo type (p<0.01). In EIC and cases of surgical margin positivity, the linear type was more frequent than the non-comedo type, but not significantly. Also the granular type showed a high positivity rate at surgical margins. However, there were no distant or local recurrences in any of the cases. It is concluded that detection of mammographic calcifications may be helpful for predicting comedo -type cancer. As mammographic calcifications might indicate high-grade cancer spread this group of patients should undergo extensive wide excision and appropriate adjuvant radio-humoral therapy.
