The results of a mass screening program for breast cancer by physical examination carried out from 1983 to 1995 in Zentsuji City were as follows : 1) A total of 18, 619 women were examined during these 13 years, including 5, 748 initial examinees (30.9%) and 12, 871 (69.1%) periodic examinees. The proportion of those aged 50 years and over was 50.8%. 2) The annual ratio of total examinees to the subject population (the coverage rate) has been 20.2% to 30.4% since 1989. Of the total 18, 619 examinees, the screening results showed that 1, 439 (7.7%) required further examination, of whom 1, 435 (99.7%) underwent further examination. Finally, 22 breast cancers and 8 interval breast cancers were detected by this mass screening, giving a 0.12% detection rate. 3) The rates of early-stage disease and cases with negative nodal involvement among cancers detected by mass screening were 31.8% and 59.1%, respectively. For these cases, the 10-year survival and disease-free survival rates were 87.8% and 78.2%, respectively. 4) When 8 cases of interval breast cancer were assessed as false negative, the sensitivity, specificity and positive predictability of this screening were 73.3%, 92.4% and 1.53%, respectively. The administration for mass screening in Zentsuji city was characterized as follows : 1) Attentive management using computers for the subjects and examinees. 2) Enlightenment of the target population by making use of the volunteer's knowledge and recommendation to receive further examination along with a sufficient grasp of information by individuals after screening. 3) Continuous using of individual records from the first screening. 4) Ongoing guidance of individuals by public health nurses. 5) Intimate contacts with the medical facilities conducting further examinations. 6) Feedback of data from further examination to the screening doctors in charge. Thus, comparatively accurate data were obtained from the present screening, although the outcome of breast cancer detected by this system was similar to that in reports of breast cancer screening by conventional physical examination.
