Direct and Indirect Effects of Screening for Breast Cancer by Inspection and Palpation at Nara National Hospital During a 5-Year Period.

Mass screening for the breast cancer involves inspection and palpation, and additional mammography in some cases. Mammography is an effective method for detecting early-stage (T1) breast cancer but is very expensive in terms of both technical labor and purchase of radiation equipment. Because of this, mass screening with inspection and palpation are standard methods, in Nara prefecture, Japan in 1997, and mammography has not yet been introduced. Examination of breast cancer in Nara city, furthermore, is not based on groups but on individuals.<BR>Individual breast cancer screening with inspection and palpation began in Nara National Hospital as one of the hygiene services for citizens in Nara city in 1992. This not only needed no other expenditure but also brought further income and effective usage of facilities to the hospital. In the five-year term from 1992 to 1996, 28, 671 women received this service. Initial screenees decreased as the number of recruits increased, though 1, 300 women were examined for the first time in the first year. In this period, 95 breast cancers were found (0.33%), among which 51 (53.7%) were detected as T1 size and 32 were persons who were repeatedly examined had 27 of the T1 lesions (84.3%). With the increase in the number of Ti lesions annually breast-preserving surgery has become more gradual (40% in 1992, 61% in 1996). Forty-three womens with 95 breast cancers were treated with this therapy.As a result, surgeons who engage in breast cancer therapy must make efforts to detect T1 breast cancers to improve curativity and the patients' quality of life with repeated examinations using inspection and palpation, at least until mammography can be introduced.
