Screening for breast cancer has been conducted by physical examination with mammography in Tokushima Prefecture. The number of subjects who were screened between October 1992 and March 1996 was 2, 500. Breast cancer was detected in 15 cases (0.6%). Mammography was able to detect the cancer in 14 cases, and 12 cases of these were at an early stage. During the period 1992-1994, the screening subjects were limited to women more than 50 years old. Then in 1995 the age coverage was changed to subjects older than 40 years. The film was also changed to a high-contrast type, which is more suitable for women with densely shadowed breasts. The mammographic parenchymal patterns (Wolfe's classification) of subjects in their forties were compared between the period 1992-1994 and the year 1995, and it was found that the incidence of a DY pattern decreased from 14.3% to 7.9%. Mammograms of 929 cases screened in 1995 were their reviewed. Most of the mammographic images for women in their forties (63.9%) were classified as P2 pattern mammograms. Therefore, when mammography screening is introduced for women in their forties, it seems important to detect tumors in P2 pattern mammograms, where the density of the mammary glands is relatively high, indicating that the accuracy of mammography should be further improved. A review of mammographic images in the mediolateral oblique (MLO) and craniocaudal (CC) views indicated that local high-density areas were distributed mostly in the central or middle to upper-lateral part of the mammary gland. These results suggest that the receiver unit of the photo timer should be positioned in the slightly upper part of the breast including the middle part for MLO and in the slightly lateral part including the middle part for CC.
