We have studied the introduction of screening mammography (SMG) from the standpoint of the current situation and a model program. <BR>For the last five years, our breast cancer screening program in Hokkaido has yielded a detection rate of 0.18% on average, which is significantly higher than the figure of 0.08% for the rest of Japan. This is partly because of the high number of deaths due to breast cancer in Hokkaido, but also because the rate of accurate screening is increased by conducting secondary examinations with mammography simultaneously with the primary examination. For this reason, the introduction of mammography into primary screening is useful for expanding the screening program as well as for accuracy. We have learned through our model program that shifting from the current system to SMG presents a number of problems. Particularly, the management system and techniques for SMG imaging should be conducted under satisfactory supervision and instruction. In order to encourage breast cancer screening by SMG, it is hoped to establish a technical guidance system and training of technicians as soon as possible.
