In Ojiya-city, Kita-Uonuma-gun, Niigata Prefecture, with a population of 32, 871 women aged 30 years and over, screening for breast cancer has been conducted since 1979 with stress placed on doctors' lectures and practice for acquiring skill in breast self-examination. On the basis of these experiences and results, it was investigated whether breast self-examination by patients could be substituted for breast inspection and palpation by doctors, focusing on the following aspects :
1) How can knowledge of breast self-examination be spread?
2) Can lesions be discovered by breast self-examination?
3) Is early diagnosis by breast self-examination possible?
4) Investigation of the examinees' awareness about breast self-examination and breast inspection and palpation by doctors.
5) Thoughts of public health nurses and hospital nurses about the effectiveness of breast self-examination.
The results obtained demonstrated the usefulness of breast self-examination. However, in a real situation, there seems to be resistance to exchanging breast inspection and palpation by doctors for breast self-examination by patients. The spread of breast self-examination has been promoted, and this has been used to supplement the role of the doctor. However, there is a body of opinion that still considers the role of the doctor essential.
