Results of breast screening by ultrasonography alone in Nagoya from 1985 to 1995 were reviewed. Of 228, 138 women screened, 133 carcinomas were detected within 10 years of screening, including 31 breast cancers which showed apparently normal findings on ultrasonograms at the time of diagnosis. Retrospective review of the screening ultrasonograms for these 31 interval cancers indicated that 8 of the tumors were missed (observer's error), 10 studies showed no tumor (true interval carcinoma), and 13 studies showed subtle signs of malignancy (unrecognized mass). Our US technicians-had tried to decrease the number of unrecognized breast cancers due to poor-quality US imaging, but their efforts were not rewarded. Screening of many participants with high-attenuation breasts and without satisfactory scanning of the areolar tissue, including assymmetric breast tissue, may reveal a higher rate of suspicious malignancy, and these cases may be truely false-positive after further examinations.<BR>We emphasize that in our experience, no breast cancers detectable by whole breast water-path ultrasound scanning were not palpable. Therefore, we should replace our dedicated automatic US system with mammography or real-time ultrasonography in order to detect nonpalpable breast cancer.
