Efficiency of Cytology for Breast Carcinomas without Evidence of Malignancy on Imaging Diagnosis.

We report the efficiency of cytology for breast carcinomas showing no evidence of malignancy on imaging diagnosis. Between January 1989 and December 1994, mammary cytologic examination was performed on 2, 741 patients at Kurume University Hospital. The cytologic diagnosis was Class I in 1, 470 cases (53.6%), Class II in 131 (4.8%), Class III in 59 (2.2%), Class IV in 41 (1.5%), and Class V in 224 (8.2%). Histologic examination was performed in 126 Class I and II cases (negative cases), and as a result 8 cases of breast cancer, comprising papillo-tubular carcinoma (5 cases), scirrhous carcinoma (2 cases) and lobular carcinoma (1 cases) were confirmed. Among 228 Class IV and V cases (positive cases), 9 cases of benign lesions, i.e., fibroadenoma (3 cases), intraductal papilloma (4 cases), intracystic papilloma (1 cases) and benign stromal tissue (1 cases) were confirmed histologically.<BR>The accuracy of cytodiagnosis was 93.7% for negative cases and 96.0% for positive cases.<BR>In addition, of 324 cases diagnosed as Class III or over, 243 were confirmed to be breast cancer histologically. Seventeen (7.3%) of these 234 breast carcinomas, which did not exhibit specific mammographic and ultrasonographic findings suggestive of malignancy, but in which preoperative cytologic examination suggested malignancy, were examined morphologically.<BR>The 17 cases comprised of 8 papillo-tubular carcinomas, 4 mucinous carcinomas, 2 scirrhous carcinomas, 1 solid-tubular carcinoma, 1 intracystic papillary carcinoma and 1 non-invasive ductal carcinoma.<BR>Eight of the 17 tumors were less than 1.0 cm in size, and 8 measured 1.1-1.5cm.Characteristically, none of the mucinous carcinomas less than 1.5 cm in size showed evidence of malignancy on imaging diagnosis.
