The Characteristics and Problems of Interval Breast Cancer.

The present study addressed the characteristics and problems of interval cancer, which was defined as follows :<BR>1) Cancers found by methods other than screening <BR>2) Cancers found within two years after screening. <BR>Breast cancer screening was carried out in local districts of Hokkaido between 1979 and 1994. Primary screening was performed by inspection and palpation. Subsequent screening revealed 18 cases of breast cancer in symptomatic women, and interval cancer was reported in nine cases after the screening. These 27 cases were examined in the present study. <BR>The clinical stages of interval cancer were Tis (one case) and II (eight cases).With regard to nodal status, seven cases showed n0 and two cases n1α. The symptomatic cancers detected at the subsequent screening included ten cases (55.6%) at stage II or higher, and six cases (33.3%) at n1b or n2. In these symptomatic women, no abnormality had been found on the previous screening, and they undertook the screening despite being aware of a breast mass. If these patients had visited medical facilities, they would have been classed as having interval cancers. The characteristics of these symptomatic cancers appeared to be similar to those of interval cancer. However, there were differences in histological type and growth appearance between these two types of cancers. More than half of the interval cancers showed expansive growth, whereas more than half of the symptomatic cancers showed intraductal spread. This difference in the growth pattern suggested that interval cancers would be perceived fairly abruptly by the patients. <BR>The occurrence of interval cancer is one of the parameters used for evaluating the accuracy of mass screening. Under the present screening system in Japan, the occurrence of interval cancer seems to be related to particular growth features and the degree of expertise of the mistakes of screening physician, as well as the rapid growth of the cancer. Therefore, a health education campaign and improvements in screening methods, including the introduction of mammography, should be considered.
