Mass screening for breast cancer using clinical breast examination with mammography has been carried out for women aged 50 or over since December, 1989 in Miyagi Prefecture, Japan. Among 23, 783 women screened, 797 (3.35%) were recalled and 54 (0.22%) breast cancers were detected up to March, 1994. Three interval breast cancers were found after screening. These were a stage Ma cancer diagnosed 7 months after screening and two stage I cancers diagnosed 20 and 23 months after screening. The sensitivity, specificity and positive predictive values of the screening were 98.2%, 96.9% and 6.7%, respectively. On the other hand, among 82, 568 women screened by clinical breast examination alone, 3, 551 (4.30%) were recalled and 82 (0.10%) breast cancers were detected in the same period. Fourteen interval breast cancers were found, giving sensitivity, specificity and positive predictive values of 85.4%, 95.8% and 2.3%, respectively. We conclude that clinical breast examination with mammography decreases the incidence of interval breast cancer and is more appropriate for breast cancer screening than clinical breast examination alone.
