We report a case of granular cell tumor of the breast detected as a spiculated mass by mammography. The patient was a 36-year-old woman who complained of a breast mass. Mammography revealed a category 5 spiculated mass. Ultrasonography (USG) demonstrated a well defined coarsely hypoechoic lesion highly suspicious of solid tubular carcinoma. MRI revealed a highly intense coarse mass in the early phase, corresponding to invasive breast cancer. However, examination of a core needle biopsy sample revealed a diagnosis of granular cell tumor. Endoscopic tumor extirpation was performed, as in a case of malignancy. Granular cell tumors of the breast are sometimes misdiagnosed as breast cancer due to the similarity of their features revealed by clinical examination, mammography and USG. To avoid misdiagnosis, core needle biopsy has been recommended as a reliable method for accurate diagnosis of granular cell tumor. While the majority of cases of granular cell tumor are benign, as in this case, a few malignant cases have been reported. Therefore, careful follow-up is mandatory, even if complete resection is achieved.
