To consider the optimum method of breast cancer screening for HRT patients, we reviewed the findings of breast imaging between July 2004 and January 2006.
Forty-six patients who underwent HRT for more than 6 months (HRT group : age 54.0±1.27 years) and 120 postmenopausal patients aged younger than 65 years without HRT (non-HRT group : age 53.7±6.72 years) were studied. The HRT regimens comprised estriol therapy in 5 patients, estradiol therapy in 16 (E2 group), and estradiol-progestin therapy in 25 (EP group). Two-direction mammography (MMG) and ultrasonography (US) were performed for all patients as cancer screening.
A heterogeneous and high-density pattern was identified by MMG more frequently in the HRT group than in the non-HRT group (85%vs70%, p=0.004), and in the EP group more frequently than in the E2 group (96%vs69%, p=0.05). Cyst formation was recognized by US more frequently in the HRT group than in the non-HRT group (26%vs13%, p=0.03).
In conclusion, the changes in breast imaging due to HRT can be detected with MMG and US, especially in women receiving estradiol-progestin treatment. US as well as MMG are necessary in order to detect early changes and should be carried out more frequently than usual after starting HRT.
