:A Questionnaire Investigation Conducted in 2003 and 2005

In 2003, we published a report entitled "Common Understanding of Women about Breast Cancer and their Competence in Self-examination : A Questionnaire Investigation". Thereafter, in April 2004, the Ministry of Health, Welfare and Labor issued a New Guide for Breast Cancer Examination (AANo. 0427001), recommending the introduction of mammography for breast cancer examination in women aged over 40 years.<BR>We investigated changes in the understanding and behavior of women before and after publication of these reports using a questionnaire investigation conducted in 2003 and 2005. The study subjects were women aged 30 to 69 years living within 30 km of metropolitan areas. The number of women surveyed was 352 and the questionnaires were sent by mail. Answers were received from 331 (94%) of the women, which was as high a participation rate as before. Understanding about the importance of mammography for screening breast cancer was increased greatly to 74.3% (48.3% in 2003), but women still chose to visit obstetricians/gynecologists (56.5%) and breast surgeons (37.2%).<BR>As a whole, the percentages of breast cancer examinations conducted using various methods were : mammography 12.4% (5.8% formerly), echography 10.3% (10.5%), palpation 29.9% (29.2%), self-examination 37.8% (39.1%). The main complaints were pain after mammography in 34.1% (10.5% formerly), and a preference for female technicians in 48.8% (36.8%). Before mammography examination, 24.4% of the women had not received any explanation about the procedure. The reasons why women did not undergo mammography were, in order of frequency : no opportunity to do so 46.9% (39.3% formerly) lack of symptoms 34.8% (40.0%); lack of awareness about where to go 24.8% (26.2%); possible expense 17.6% (10.8%); lack of available time 16.2% (16.7%). These findings indicate changes in both the knowledge and behavior of women in relation to breast cancer examination, and suggest that in Japan more efforts should be made to improve the understanding of women about breast cancer and to ensure that all are given a chance to receive breast cancer examination, including mammography, anywhere at any time. Only this approach will reduce mortality due to breast cancer.
