Since precise examination using stereotactic needle biopsy is available at only 2 of 77 institutions in our district, we attempted to evaluate our current diagnostic procedures for lesions showing microcalcification detected by mammographic breast cancer screening.<BR>A total of 19, 477 women in our district underwent breast cancer screening with mammography in 2001. The subjects of this study were 125 women whose mammograms showed category 3-5 calcifications without a tumor shadow, which were detected by a double check system at the screening. The number of women whose calcifications were classified as category 3, 4, and 5 was 102, 18, and 5, respectively. The number and the rate (the positive predictive value) of breast cancer detection in each group of women were 6 (6%) in category 3, 6 (33%) in category 4, and 5 (100%) in category 5. The rate of biopsy performance in each individual category was 22% in category 3, 78% in category 4, and 100% in category 5. The method of biopsy performed was surgical biopsy in 12 women, stereotactic needle biopsy in 7, and fine needle aspiration biopsy cytology in 24. Breast cancer was diagnosed in a total of 17 women, of whom 7 underwent surgical biopsy, 4 underwent stereotactic needle biopsy, and 6 underwent fine needle aspiration biopsy cytology.<BR>Thus surgical biopsy yielded a diagnosis of breast cancer in 7 (58%) of the 12 women who underwent the procedure, implying that surgical biopsy was performed judicially during our breast cancer screening. In order to avoid overuse of surgical biopsy, the indications for biopsy and a suitable biopsy procedure should be selected according to the grade of calcification.
