Background and Objective : Mammary gland cancer that is shown only as microcalcification (MC cancer) by mammography is either the comedo type or the non-comedo type, and each type has different clinical characteristics and treatment. The present study was done to examine the diagnosis and treatment of both types.<BR>Materials and Methods : One hundred and two cases of MC cancer were examined with reference to the make-up of the mammary gland in relation to calcification, cancer infiltration and possibility of spread to the lymph nodes, progression of calcification and structural progression, and the results of conservative surgery.<BR>Outcome : Comedo-type cancer was distributed with a linear/branching shape in 77% of cases, and with a pleomorphic shape in 33%. The distribution was grouped in 32%, segmental in 53%, and linear/branching while segmental in 92%. Among cases of the comedo type, 51% showed infiltration, of which 23% were confirmed to have spread to the lymph nodes. The corresponding figures for the non-comedo type were 19% and 9 %, respectively. The correlation between calcification progression and structural progression was 0. 9 for the comedo type and 0. 6 for the non-comedo type. When the progression of calcification was less than 25mm, the success rate achieved with conservative therapy was 85% for the comedo type and 70% for the non-comedo type.<BR>Conclusion : It is possible to predict the extent of progression of MC cancer and its make-up on the basis of calcification, and also determine the propriety of conservative therapy. If there is a possibility that the cancer is of the comedo type, a prompt diagnosis is imperative.
