Purpose : We studied the characteristics of breast cancers that cannot be detected by mammography (MMG) and evaluated the importance of palpation in breast cancer screening.<BR>Subjects : Between 1999 and 2000, 987 cases of palpable breast cancer were treated surgically at the Cancer Institute Hospital (excluding post-biopsy cases, male breast cancer, and bilateral breast cancer). Of these cases, we studied 70 that could not be detected by MMG screening (MLO view).<BR>Results : The incidence of palpable breast cancers that were undetectable by MMG was 7.1% (70 /987). The incidence was 3.7% (22/595) in women aged 50 or over, and 12.2% (48/392) in those aged 49 or under. These 70 cases were not all early cancers : 43% were over 2 cm in diameter, and 83% were invasive. Most of them were located in rather dense breast tissue, and no cases in fatty breast were overlooked. Among these cases, 62% were detected by the patient herself, 24% by the doctor upon palpation, and 14% by US.<BR>Discussion : Screening MMG without palpation sometimes fails to detect palpable breast cancer, and the incidence of such cases is higher in women aged 49 or under. These are not all early cancers ; T2 or invasive cases may be overlooked. Education of women about breast self-examination is very important because about 60% of these cases were detected by the patient herself. However, palpation during breast cancer screening is also important because about a quarter of the present cases were detected by the doctor upon palpation.
