[Objective] To evaluate the problems of omitting physical examination (PE) in screening for breast cancer using ultrasound (US) combined with mammography (MMG).<BR>[Subjects and Methods] The proper combination for screening modality was investigated in 420 cases of breast cancer diagnosed in outpatient sections from January 2000 to December 2001. Breast cancer detection rate, cancer histopathological type and lymph node status were investigated for each combination.<BR>[Results] The breast cancer detection rate was 80.5% with PE only ; 85.2% with MMG only ; 92.6% with US only ; 90.5% with a combination of PE and MMG ; 96.7% with a combination of PE and US ; and 98.6% with a combination of MMG and US. Forty (9.5%) out of 420 cases were not detected by PE combined with MMG, and 30 (75%) of these 40 were invasive cancer. Fourteen cases were not detected by PE combined with US, and six (42.9%) of these 14 were invasive cancers. Six cases not detected by MMG combined with US included five with symptomatic nipple discharge and one symptomatic Paget's disease, and all were non-invasive cancers.<BR>[Conclusion] PE can be omitted in breast cancer screening using a combination of MMG and US only if the examiner checks for the presence of nipple discharge.
