At the Miyagi Cancer Association, work-up for suspicious lesions was performed by diagnostic mammography and ultrasonography immediately after breast cancer screening. We evaluated the accuracy of this work-up and the recommendations made in the place where the breast cancer screening was carried out. The screening involved a clinical breast examination with or without mammography, and the screening interval was two years. We investigated the work-up and the recommendations for 230, 979 women who underwent breast cancer screening between 1995 and 1998. The same investigations were done for 306 screen-detected breast and 25 interval cancers, that were detected within a year, between 1996 and 1999. There were 7, 531 recommendations (3.3%) that the screenee should revisit the screening after one year. Such recommendation was made for thirteen screen-detected cancers and five interval cancers. Among the eighteen patients, thirteen (72%) had palpable breast masses, fifteen (83%) were younger than 50 years old, twelve (67%) were diagnosed as having mastopathy and six (33 %) were diagnosed as having cancers by re-evaluating the mammography and ultrasonography examinations. On the assumption that the five interval cancers were false negative cases in the work-up, the sensitivity of the work-up was 98.4%. Observation for one year is inadequate for a screenee who has a palpable breast mass. Quality control for the work-up after breast cancer screening is therefore necessary.
