Effective breast cancer screening, by both physical examination and mammography, requires both high quality films and their accurate interpretation. A lack of doctors qualified to read mammograms prompted various groups to hold training courses on mammogram interpretation, under the joint auspices of the Central Committee for Quality Control of Mammographic Screening (CCQCMS). Unfortunately, performance on post-course tests was worse than the pre-course performance. In an attempt to improve instruction and reconsider the content and methods of the training courses, the CCQCMS sent questionnaires to instructors. Subsequently, the course participants' post-course test results improved. Evaluation was based on the results of the Osaka training course held by the Ohuchi Study Group in March 2000. Twenty-nine training courses (1412 participants, 30 post-course test groups) before March 2002 were divided into three periods : 8 courses between March 2000 and January 2001 (period 1), 11 courses between February and September, 2001 (period 2), and 11 courses after October 2001 (period 3). Over 50% of α-ranked course participants were expected to achieve post-course test scores of A (instructor level) or B (adequate level) while for β-ranked courses the expectation was over 70%, and for γ-ranked courses (experienced), over 90%, with over 30% scoring A. The quality clearance for each period was calculated as the proportion of all courses during the period that satisfied or nearly satisfied this expectation. In period 1, the quality clearance was 88%, and in period 2, 45%. As a result of the questionnaire to instructors and coincident improvement in the training programs, the quality clearance rose to 82% in period 3. If this method of quality improvement is continued, it will benefit future training courses.
