:Cooperation Between Local Governments and Medical Associations in Osaka Prefecture

Many local governments advocate mammography for breast cancer screening (screening mammography : SMG). Close cooperation between local governments and medical associations may improve breast screening programs, particularly with respect to quality control issues. However, participation of local medical associations in SMG differs greatly among local regions. In Osaka Prefecture, SMG has been adopted by only a few local governments. To investigate this, we sent questionnaires to health centers in 42 local government regions in Osaka Prefecture. Answers from 35 health centers were available for analysis. In 2001, SMG had been adopted by 10 of them (29%). No medical association participated in SMG or had been invited to evaluate the quality of service in conjunction with the local government. Ten other local governments planned to adopt SMG in the near future. The results indicate that in Osaka Prefecture, local governments and medical associations neither cooperate in delivering SMG nor do they have quality surveillance programs, thus diverging from the SMG guidelines. In Osaka Prefecture, 130 doctors have already participated in mammography interpretation training courses. These doctors and other practitioners must play a more visible role in SMG and the Breast Cancer Committee of the Adult Diseases Instruction and Control Council must be activated in order to optimize patient care.
