Evaluation and Significance of Soft Neurological Signs in Pediatric Neurology

The term "soft neurological signs" is usually applicable to the signs of minimal brain dysfunction, which may not be demonstrated by classical neurological examinations. Especially, the signs of minor motor dysfunction are stressed, but those of perceptual disorders, language disabilities, or behavior disorders should be also included in soft neurological signs.<BR>The signs of minor motor dysfunction may be classified into motor incoordination, associated movement (synkinesia), choreiform movement, motor impersistence, and so on. It should be mentioned that most of them are regarded as common neurological signs in adult patients.<BR>On the other hand special neuropsychological tests such as WISC performance test, Bender Gestalt test, Goodenough Draw-a-Man test, Frostig developmental test of visual perception, and ITPA may be generally applied as the examinations of perceptual disorders and language disa-bilities in children. However, the signs of percep-tual disorders and language disabilites should be essentially placed under the category of agnosic, aphasic, or alexic defects, which are also common neurological deficits in the adults.<BR>Thus, most of the soft neurological signs may be included in general neurological signs. However, in children, they may always exist in the earlier ages, and disappear or change in the course of development. Therefore, it may be often difficult to evaluate the normal ranges of soft neurological signs, which will be discussed in this symposium.<BR>Another important problem to be discussed is that the presence of soft neurological signs do not always indicate organic brain damage. Some-times, it may merely show delay of maturation. In addition, even if organic brain damage exists, the higher cerebral functions of this kind may be compensated and tend to improve, because the functions of developing brain may often possess "plasticity".<BR>Finally, it should be also emphasized that the assessment of soft neurological signs may be related directly with the procedures of rehabilitation or educational therapy for the children.<BR>The problem of soft neurological signs should be one of the most important problems in pediatric neurology, which may be the integrations of adult neurology, and developmental neurology.
