Risk factors in central nervous system infection during childhood.

To find the risk factors of central nervous system infection, clinical records of 228 children with central nervous system infection were studied retrospectively. Patients consisted of 114 with serious meningitis, 40 with bacterial meningitis, 54 with acute encephalitis and 20 with acute encephalopathy. Sixty patients of Kawasaki disease and 60 patients of mycoplasma pneumonia were also studied as controls. The examined items were divided into two groups;(A) the factors before the first week of life and (B) the factors thereafter.<BR>There was an obvious sex difference in the incidence of central nervous system infection. Namely, male patients were found more than females in this study (156 of males and 72 of females).<BR>About the factors B (body weight and height at disease onset, mental and motor development at disease onset, and history of convulsion) no difference was found between each patient group and control groups. Concerning the factors of A, each patient group showed higher frequency of perinatal abnormalities than control groups, especially of jaundice. In the other factors of A, family history of convulsion and troubles during pregnancy were slightly more frequent in some patient groups, and showed a tendency of shorter gestational period, and lower birth weight in each patient group than control groups. But the differences were not significant.<BR>The children who developed mental retardation after central nervous system infection (5 after purulent meningitis, 6 after acute encephalitis, 2 after acute encephalopathy) had the character as follows. Mean age was younger than that of all patients. They were six males and seven females. Higher frequency of troubles during pregnancy and of perinatal troubles, shorter gestational periods, lower birth weight than all patients were noted.<BR>In conclusion, two risk factors in central nervous system infection of childhood were suggested; male sex and history of perinatal troubles, especially of jaundice.
