Computerized Image Analysis and Chronological Changes in the Mammographic Features of Microcalcifications of Breast Carcinoma.

The mammographic features of 47 breast cancer patients with clustered microcalcifications were studied in comparison with the previous survey films. Twenty seven patients had several extremely faint calcifications and their breast carcinomas were detected about a year earlier than patients who had shown no significant abnormality in the previous survey films. In seven of the 48 patients with faint calcifications, prospective study revealed the existence of breast carcinoma. <BR>Changes in microcalcifications on mammograms of breast carcinoma were studied in 18 patients. The distribution of the size of microcalcifications was also studied by computerized image analysis. The extent of microcalcification spread was clssified into two types by simple linear regression ; 1) the rapid spreading type was common in the comedo type (y=-11.94+1.27 X, R=0.73), and 2) the slow spreading type was common in the non-comedo type (y=-0.69+ 0.163X, R=0.98). There were two cases which were followed by mammography on two or more occasions, showing rapid spread in the latter period from slow spread in the early period. The distribution pattern of the size of microcalcifications also changed from the non-comedo type to the comedo type of mixed-type carcinoma. It was suggested that breast carcinoma in which intraductal spread is predominant may convert its proliferation to a more malignant grade.
