We tried to determine the optimal interval between fine-needle aspiration (FNA) and radical operation in patients with breast cancer. Fifty-nine node-negative patients, who had received FNA preoperatively, were examined. All ipsilateral axillary lymph nodes embedded in paraffin blocks were serially sectioned and stained with hematoxylin and eosin and a cytokeratin stain, CAM 5. 2. The interval between FNA and operation in five patients with micrometastasis was 13, 17, 20, 26 and 30 days, respectively. These were divided into an event group, and the rest into a censored group, respectively. The observed cumulative probability [95% confidence interval (CI)] of micrometastasis occurrence was estimated using the Kaplan-Meier method. The ratio without micrometastasis was 98% [95% CI : 94-100%] between 13 and 16 days, and 94% [88-100%] between 17 and 20 days. These results suggest that the optimal interval without micrometastasis, at least 97% or more, might be between 13 and 16 days. Therefore we conclude that surgery would be better performed within about two weeks after FNA.
