Purpose: Hand-held portable ECG recorder that uses only both hands is useful, because it can be used in the public. However, we sometimes experience difficulties especially with young women because of their low R wave amplitude (R). We aimed to improve R by changing the posture. Method: We recorded ECG with Read My Heart Plus® portable recorder with five different postures of upright sitting (U), lean forward (LF), lean backward (LB), upright standing (US) and lying spine (Sp). Results: 51 volunteers (14 male, 37 female) were enrolled. R of U and LF were the largest with 7.1 mV. Those who have smaller R are clinically important, thus we analyzed those who had smaller R less than 10 mV. Among such 40 (11 male, 29 female) subjects, 58% represented larger R with LF (5.69 mV) than U (5.39 mV), while only 49% with whole 51 subjects. Lighter subjects tended to have smaller R (p=0.0064), and so was the female than the male (p=0.0001). Conclusion: Hand-held recorder will be encouraged to use with U posture. In case of small R, LF posture may improve the R.
