Latent Early Acoustic Neuroma in the Internal Auditory Canal; Report of Three Cases.

The first patient was a 53-year-old female who was examined by Gd-enhanced MRI for the assessment of trigeminal neuralgia ; a 6×4mm tumor was found in the internal auditory canal. The second patient was a 45-year-old female whose CT scan performed because of a middle ear abnormality showed an enlargement of the right internal auditory canal. Gd-enhanced MRI incidentally revealed a 3×3mm tumor in the left internal auditory canal. These two patients had no clinical signs pertaining to their tumors. The third patient had been treated for secretory otitis media and normal hearing had been restored. Eight months later Gd-enhanced MRI was necessitated for sudden sensorineural hearing loss of moderated degree. A 6×9mm intracanalicular tumor was revealed. The tumor had apparently grown asymptomatically before the episode. Following total removal of the tumor through a middle cranial fossa approach, her hearing improved significantly. Some of the clinical aspects of latent early acoustic tumor, a new otologic entity in the MRI era, are described.
