A 5-year-old boy with congenital cholesteatoma in the mesotympanum also had external abnormalities on the involved side: bat ear and low set ear. The cholesteatoma seemed to originate from the undersurface of the processus cochleariformis and the tendon of the tensor tympani muscle.We reviewed 208 reported cases of congenital cholesteatoma of the middle ear.In 28 cases it originated from the tympanic isthmus, as formerly reported. The incidence in boys was twice as high as in girls. A relatively high incidence of congenital malformations was found. These findings suggest that congenital cholesteatoma is the result of an embryologic error.
