Experience of arthroplasty for glenohumeral osteoarthritis.

Three patients with glenohumeral osteoarthritis were treated by arthroplasty. Patients were 46, 31 and 52-year-old men. All patients suffered from severe left shoulder pain with restriction of movement and loss of upper limb function.The approach is made through the deltopectoral groove. The marginal osteophytes of the humeral head and glenoid are removed. Irregular articular surface is shaved to be smooth. The coraco-acromial ligament is excised. The plaster cast is applied to hold the arm in zero position after operation. At five days after operation, passive shoulder movements are started.The follow-up ranged from nine months to two years and one month, with a mean of one year and five months. The results were examined according to the shoulder evaluation form of Jpn. Orthop. Assoc.. The results showed relief of pain and an increase in range of movements in all patients. The average of total points was 44 before operation and 78 after operation. Reontgenograms after operation showed an increase of glenohumeral movements.We believe that this procedure should be considered before hemiarthroplasty and total arthroplasty.
