Retrospective clinical study of the treatment of elder adult patients with complete tear of the rotator cuff.

A total of 29 cases with complete tear of the rotator cuff above 50 years old were seen during the period of 43 months from February 1985 to August 1988. Fifteen cases were treated with conservative therapy and 14 cases with operative therapy. The results of these treatments and clinical feature of the cases were retrospectively studied, comparing with each group of cases, to define indication of operative therapy.There was no significant difference in the results of treatment between the two groups of cases. Many cases treated with conservative therapy recovered within the following two months after the onset. In 12 cases (86%) of 14 cases treated with operative therapy, their severe pain at night exactly indicated the operation. There were more patients who had routine occupation in the cases with non-operative therapy. No difference was seen in the past trauma between the two groups of cases.In conclusion, operative therapy might be indicated to elder adult patients with complete tear of the rotator cuff, when symptoms, especially severe pain at night, still remains under conservative therapy of two to three months duration, especially to patients who have routine occupation.
